Firstly thanks for a great plugin.
I'm using Joomla 1.0.x If I use {include_content_item n) should I get any images in the content item? I'm not getting any but I'm not sure if I should.

Are you using the "mosimage" to include the images?
If yes, please go to the Plugin/Mambot Manager. I think most likely your MOS Image plugin is ordered in front of Include Content Item plugin.
Try moving the MOS Image plugin below the Include Content Item plugin so that the Include Content Item is being processed first followed by the MOS Image plugin.

Which one comes first in the order: the multithumb plugin or the Include Content Item?

Include Content Item comes before Multithumb. If I put multithumb in front of Include Content Item then I see a multithumb tag e.g. {multithumb thumb_width=200} in the content item text.

Can you try unpublish Multithumb and see if it works with just "Include Content Item" and "Mosimage".
If it works, then it might be the problem with the Multithumb plugin. I've seen some plugins designed not using the standard way and as such will not work with other plugins.
